Introduction
Inflammation is behind many common health problems. You might be familiar with joints that feel stiff, airways that tighten, or maybe your gut flares up after stress or illness. And yes, inflammation could be behind all of these. So, do you take an anti-inflammatory? It sounds obvious put that way, but what most people don’t realise is that not all anti-inflammatories work in the same way. And, I’m not just talking about the difference between prescription anti-inflammatories and natural remedies.
In this post, I explain how fish oil, turmeric and boswellia each work in different ways, and importantly at different points in the inflammatory process. And, that can make a lot of difference to your comfort today and recovery over time.
Let’s look at two broad phases of inflammation. The first phase responds to a threat. Heat, swelling and pain arrive to protect and repair. Then, the second phase clears up the mess and restores calm. Trouble starts when the first phase runs too hot or too long, and the second phase struggles to finish the job. In herbal medicine, what we’re looking for are ways to help the body do what it needs to do. To moderate the first phase. But also to help the body clear up.
This guide explains how each remedy supports that aim, when combining them makes sense, and where food choices can also help.
How inflammation works in simple terms
Your body is made up of cells. Think of your cell membranes as a store cupboard of fats. When tissue is under stress, the body opens that cupboard and takes out a fatty acid called arachidonic acid. That fat can then follow two main routes.
One route uses enzymes called cyclo-oxygenase or COX. This leads to small chemical messengers that widen blood vessels and heighten sensitivity. You feel heat and pain.
The second route uses an enzyme called 5-lipoxygenase, or 5-LOX. That makes messengers called leukotrienes. Leukotrienes pull more white blood cells into the area. They can also tighten airways and increase mucus.
Many pain relieving medications block the COX route. Pain can ease. Yet if the COX route is blocked too strongly, more of the stored fat can move down the 5-LOX route. This is called a “shunt.” Leukotrienes can then rise and keep tissues irritated. You might notice chest tightness or gut upset in this situation.
There is also a clean-up phase. The body makes signals that tell immune cells to tidy up and then calm down. When resolution works well, swelling settles and tissues repair. Fish oil supplies the raw materials for those signals. Turmeric helps reduce the messages that keep pushing inflammation forward. Boswellia blocks the leukotrienes that add fuel to the fire.
Fish oil: changing cell membranes
Fish oil contains the long-chain omega-3 fats EPA and DHA. These sit in your cell membranes and change the mix of fats there. When the body reaches into that store cupboard, it pulls out EPA and DHA as well as arachidonic acid. The result of having these omega-3 fats as well, means there is a different blend of signals.
From EPA the body makes gentler versions of those inflammatory messengers. They are less likely to drive sharp pain or heavy swelling. EPA and DHA also aid the clean-up phase. Your body turns them into signals that guide immune cells to clear up debris and then switch off. This is not suppression of inflammation. This is finishing the job properly.
EPA and DHA act on switches inside the cell that control inflammation signals. That helps to turn them down. They also make the cell membrane more fluid, so the stress sensors on it are less easily triggered.
An important point to note is that fish oil doesn’t act immediately. It can take weeks for these important fats to be incorporated into enough cell membranes to have an effect. Over time you may notice easier mornings, fewer delayed aches after activity, and easier breathing in cold weather.
Food can be a strong base. Eating oily fish two to three times a week can lift your levels. If you rarely eat fish, a concentrated fish oil can help you raise EPA and DHA further.
Turmeric: turning down the master switch
Turmeric root contains curcumin. It’s curcumin that helps to turn down the control switch inside cells that turns on many inflammatory genes. When that switch is overactive, tissues keep making inflammatory enzymes and signals. Curcumin helps hold that switch in the resting position.
Curcumin also reduces the cell’s production of a key enzyme that makes many painful chemical messengers. It can slow the first step that releases arachidonic acid from the membrane. That means less fuel for the two key pathways, COX and 5-LOX. If less is released, there is less to convert, so fewer prostaglandins and leukotrienes are produced.
Alongside this, curcumin supports the body’s own antioxidant systems. It helps mop up irritating by-products made during a flare. Day to day, you may notice less stiffness on waking, easier flow through repetitive tasks, or a shorter flare after a knock.
Curcumin has been known to absorb poorly in its plain form. Modern extract formats improve its uptake. Taking it with a meal that contains fat helps as well. Cooking with turmeric also supports on a background level, though kitchen amounts are not the same as concentrated extracts.
Boswellia: blocking the leukotriene pathway
Boswellia is a tree resin, commonly called Indian frankincense. I am referring to the resin extract, not the essential oil. Its boswellic acids, especially AKBA, act on the 5-LOX enzyme. That enzyme makes leukotrienes. When leukotrienes are high, more white cells move into tissue. Airways can tighten. Mucus can increase. Joints can feel more irritated.
By reducing 5-LOX activity, boswellia lowers leukotrienes such as LTB4 and the airway-tightening group often called cysteinyl leukotrienes. Boswellia also helps limit the activity of an enzyme called leukocyte elastase. That enzyme can break down tissue structures when it is too active. The combined effect is less pull of white cells into tissue and less damage from the cells that do arrive.
In practice, this commonly shows up as easier breathing in weather or seasons that once were a struggle. Or, if you’re often troubled by joint stiffness, you might notice easier movement during day-to-day tasks. Boswellia is fat-loving. Take it with a meal that contains fat for better uptake. Some formulations can also improve absorption. Ask a medical herbalist for advice.
Why combinations can help
Putting these three remedies together can make a lot of sense because they work on inflammation in different places and ways. Fish oil works in several ways to lessen inflammation and helps with its resolution so that inflammation does not become chronic. Turmeric calms the gene switching that pushes inflammation. And, Boswellia blocks the leukotrienes that often surge when the COX route is reduced. When the COX route alone is blocked your pain may ease with the medication yet your chest or gut might feel unsettled. That can hint at a shunt down the leukotriene route. And adding boswellia may help cover that path.
There is also a practical bonus tp the combination. Curcumin and boswellic acids are fat-soluble. Taking them with fish oil can help them dissolve in the gut. Better dissolution often means better absorption. Over time that can mean more reliable results.
So, the three together can support each other’s strengths. Turmeric and boswellia offer shorter-term comfort for joints and airways. While fish oil builds efficacy in the background and can help flares to resolve.

Airways and seasonal tightness
When your airways feel tight with cold air or a lingering virus, leukotrienes are often involved. Boswellia targets that specific route directly. Fish oil supports the resolution phase, which matters when irritation lingers after a virus. While turmeric adds control at the gene level, and may reduce the number and strength of triggers.
The gut lining
Some gut flare ups involve leukotrienes in the bowel wall. Boswellia’s 5-LOX focus can make sense here. And, turmeric is often chosen to support a less inflamed gut lining. Fish oil then helps to move the situation from flare up to repair.
Whole-body, low-grade inflammation
When you feel heavy after meals, ache after light effort, or struggle with brain fog, fish oil is a natural base. It remodels cell membranes and promotes the resolution signals. Again, turmeric adds the gene level anti-inflammatory support. And, if the airways or your gut are also part of the picture, boswellia can be added in.
Auto-immune patterns
When the immune system overreacts, the aim is to calm that reaction. Fish oil helps to complete the inflammatory response properly. And once again turmeric reduces the push from genetic level signals. While boswellia helps to limit the leukotriene-driven tissue irritation. Because patterns vary, tailored advice is always wise.

Food first and practical use
Everyday foods that may help
Oily fish such as salmon, sardines and mackerel can provide the EPA and DHA fatty acids. Two to three portions a week helps most people. If fish is rare in your diet, a concentrated fish oil can fill the gap. It’s one of the supplements I find myself most often recommending.
Turmeric works well in cooking. It is easy to add to lentil dishes, soups and stews, or you can even make a simple turmeric milk. Include a pinch of black pepper and a source of fat such as olive oil or coconut milk for better absorption.
Turmeric isn’t the only spice with evidence behind it. Ginger, garlic and cinnamon have all lowered inflammatory markers in trials, and I’ve written about the best natural anti-inflammatory herbs and spices.
Boswellia is not a kitchen herb. Its helpful acids are in the resin. So, we use a tincture, extract or capsule. It’s best taken with a meal for better uptake.
Forms
Curcumin, perhaps the most active constituent in turmeric generally absorbs poorly. But, modern technology has devised ways to improve absorption. So, as well as powdered or encapsulated turmeric you will find curcumin products in different forms. For instance, you might see it paired with phospholipids or in reduced particle size to improve levels. Some products include piperine from black pepper to slow breakdown, improving absorption.
Boswellia extracts that specify higher AKBA content, or use lecithin delivery, can be effective. And, even fish oil comes in different chemical forms. Re-esterified triglyceride oils are usually well absorbed and gentle. You can also find fish oil with “pro-resolving mediators” which aim to help move inflammation to resolution.
Building a plan
When building a plan, I always start with food. Add fish oil if your diet is light on oily fish. Then consider turmeric and boswellia to match your pattern. Joints and airways often benefit from the turmeric–boswellia pair. Don’t expect immediate results. These natural supports tend to show their best effects over weeks, rather than days.
